COVID-19 Vaccination Information

The Department of Health-Taylor has received our initial doses of COVID-19 vaccines.

The Executive Order 20-315 directs us to focus on the following populations during this initial phase:

  • Long-term care facility residents and staff;
  • Persons 65 years of age and older; and
  • Health care personnel with direct patient contact.  

The Executive Order also allows hospitals to vaccinate persons who they deem to be extremely vulnerable to COVID-19. 

CVS and Walgreens have contracted with the state to provide vaccinations to long-term care facility residents and staff.

DOH Taylor is coordinating with local private health care providers to begin the administration of the vaccine to their front-line health care workers.

For persons 65 years of age and older, please visit our Online Request Form to request a vaccine appointment. Once received and reviewed by our nursing staff, you will be contacted to schedule your vaccine administration appointment. Once your appointment is scheduled, please print and complete the Moderna Vaccine Screening and Consent Form and bring it with you when you come for your vaccination.

As additional vaccine is made available, the vaccines will be provided to the general population in a coordinated, orderly and efficient manner to help slow or stop the spread of COVID-19 within the community. Look for more information on vaccination clinics in the future.
